Various hormone levels were monitored during therapy, and it did not appear that there were major changes in the group as a whole (Table 2). Cortisol levels rose significantly from baseline to the first dose of yohimbine. When the hormone levels were evaluated in responders vs nonresponders (Table 3), slight differences were noted. Free testosterone levels were higher at baseline in the responders but did not increase significantly with the higher doses of yohimbine. We measured nocturnal penile tumescence with the RigiScan™ monitor, hormone profiles, answers to the Florida Sexual Health Questionnaire, and clinical responses at baseline and after two different doses of yohimbine in 18 nonsmoking men with erectile dysfunction. Of the 18 men, nine (50%) were successful in completing intercourse in more than 75% of attempts. The yohimbine responders were men with less severe erectile dysfunction as manifested by improved increased rigidity on RigiScan™ testing, higher Florida Sexual Health Questionnaire scores, and slightly higher levels of serum testosterone. Yohimbine is an effective therapy to treat organic erectile dysfunction in some men with erectile dysfunction.

All of these medicines work by relaxing smooth muscles and increasing blood flow in the penis during sexual stimulation. You should not take any of these medicines to treat ED if you are taking nitrates to treat a heart condition. Nitrates widen and relax your blood vessels. In the 1700s, pellagra was an endemic disease in northern Italy—which had not been known until maize (corn) was introduced from America. Italy gave the disease the name, “pelle agra” (pelle means skin; agra means rough). Casal had observed that patients with pellagra were all poor, subsisted mainly on maize, and rarely ate fresh meat. Because pellagra outbreaks occurred in regions of Europe where maize was a dominant food crop, the belief was that maize either carried a toxic substance or was a carrier of disease. When it was later noted that there were few pellagra outbreaks in Mesoamerica, where maize is a major food crop (and is processed), it was considered that the causes of pellagra may be due to factors other than toxins.
The response to yohimbine did not vary with patient age; the responders were 60.3 y of age vs 60.0 for the nonresponders (Table 4; P=0.106). The number of medical risk factors was slightly higher in the nonresponders (2.3 per person) compared with the responders (1.8 per person), but this difference was not significant (P=0.346). Documenting the quality of the men's erections in the office with a simple grading system showed a significant difference at the end of the study between responders and nonresponders. For the responders, the value was 3.0 compared with 1.9 for the nonresponders (P<0.001). This result correlated with the overall sexual satisfaction of patients who stated whether or not they were able to engage in regular sexual intercourse.

This study was designed to test the hypothesis that hydrochlorothiazide a diuretic used to treat hypertension depletes body zinc and thereby cause sexual dysfunction. Serum zinc and sexual dysfunction were measured in 39 middle aged hypertensive men who had been taking hydrochlorothiazide in average daily doses of between 25 and 50 mg daily for at least six months, and a control group of 27 unmedicated middle aged normotensive men. The medicated group had a higher incidence of sexual dysfunction (56 pc) as compared to 11 pc in the control group. The use of hydrochlorothiazide did affect serum zinc levels significantly in 20 patients. Sexual dysfunction occurred more often in older and overweight patients (p < 0.004). Three of the normotensive men experienced sexual dysfunction probably related to old age. Twenty two of the 39 on hydrochlorothiazide and experiencing sexual dysfunction were divided into two groups of 11 patients. Bloods were taken from the 27 normotensive and 22 hypertensive men receiving hydrochlorothiazide for the analyses of zinc. Subsequently one group of the patients were supplemented with zinc 500 mg daily for 30 days while the other group was supplemented with magnesium chloride 1 g daily for 30 days. The normotensive men were not treated. After 30 days, bloods were again taken from the three groups of analyses for zinc and magnesium. Serum zinc was significantly decreased (p < 0.05) by hydrochlorothiazide and a non significant decrease in serum magnesium (p = ns) was observed. After supplementation with zinc, the serum zinc levels returned to normal only in eight patients. There was improvement in the symptoms of sexual dysfunction in five patients. Two patients gained weight. Hydrochlorothiazide decreased serum zinc levels (p < 0.05) and was unchanged with magnesium supplementation but the serum magnesium returned to normal values. Improvement of symptoms of sexual dysfunction was positive in one patient. This study shows that low serum zinc levels may be associated with sexual dysfunction but the definitive role of zinc in the pathogenesis of sexual dysfunction will remain controversial.
There is no guaranteed, permanent way to ‘cure’ ED – partly because, age is a factor that affects ED the older you are, the more likely you are to get ED symptoms. Men With Bad Lipid Readings.  One study examined men with both erectile dysfunction and "dyslipidemia."  Dyslipidemia is medical speak for bad HDL, LDL, triglyceride or some combination of the three.  They gave these men 1.5 grams of niacin, which is a megadosed amount, and is a favorite of Dr. Davis.  (See my  Review of Track Your Plaque for Dr. Davis' approach to plaque regression.) Penile erection is a hemodynamic process involving increased arterial inflow and restricted venous outflow, coordinated with corpus cavernosum and penile arterial smooth muscle relaxation. Any problem in this mechanism results in Erectile Dysfunction and its etiology is generally multifactorial. This study is aimed at determining the objective outcome of aerobic training in the management of Erectile Dysfunction of arterogenic origin using Meta analysis.
In the Hong Kong study, the researchers postulated that niacin might be as beneficial as statins on erectile function, and have other related benefits too. Niacin is known to produce a flushing effect (see “Toleration Despite Adversity,” above), which is related to prostaglandin D2 (PGD2) release in the skin. This can lead to vasodilation and concomitant flushing. The production of PGD2 can also occur in macrophages, a type of protective white blood cell. Consequently, when PGD2 production is induced by niacin, it may affect all body tissue, including the cavernosal tissue in the penis. Indeed, PGD2 is one of the potential agents causing the vasodilation and engorgement of cavernosal tissue, thereby leading to erection. Thus, niacin improves erectile function by stimulating the production of PGD2.
In male rats, main olfactory epithelium (MOE) exerts an important role in regulating sexual behavior. Intranasal irrigation with zinc sulphate has been reported to destroy the MOE and completely abolish the sex behavior.[6] In this study supplementation of zinc was done using a feeding tube and precautions were taken to avoid contacting nasal area. Hence the possibility of reducing sexual performance due to MOE disturbance is ruled out. Some humans experience gastrointestinal irritation with supplementation of zinc.[23] If the same is applicable to animals it may be another possible explanation for the reduction of libido index with elevated doses of zinc. One drawback of our study is that we did not compare the weight of animals before and after treatment.
According to a study conducted by the University of the West of England, Bristol, pelvic floor exercises may be very effective in treating erectile problems. Many participants of the study, all of whom were men who had been experiencing erectile dysfunction for at least six months, reported a significant improvement of their condition within three months of training their pelvic floor. 
Suggested intake: “The problem with yohimbine is that you can’t regulate the amount in a dosage,” says Fratellone. “It all depends on what part of the tree it comes from, how it’s cultivated, how it’s exported, and so on. The amount of extract you get will vary.” FDA researchers analyzed a number of over-the-counter yohimbe bark products. They found that the supplements contained only seven percent or less of the amount of yohimbine that would be found in actual yohimbe bark, which suggests that they contained little or no yohimbe. However, the prescription form of yohimbine is strictly regulated by the FDA. It is approved only for the treatment of impotence, and is available in tablets and capsules. For erectile dysfunction, 5.4 to 10 milligrams three times daily has been studied and regarded as generally safe.

It is now thought that ED is part of the cardiovascular disease complex related to metabolic syndrome (MS). Although endothelial dysfunction and atherosclerosis are believed to be part of the main mechanisms for ED in patients with MS, other mechanisms account for ED in MS include androgen deficiency, drugs, the veno-occlusive mechanism, etc. Because dyslipidemia is one of the key risk factors for the development of endothelial dysfunction and atherosclerosis in MS patients, there is a close relationship between ED and dyslipidemia.
Exercise lowers blood pressure – a consistently high blood pressure (hypertension) can damage your arteries and cause them to harden, limiting the flow of blood to the penis and leading to cardiovascular problems. Exercise works to reduce the force required to pump blood around your body, protecting your artery linings as a result. You should aim to have a blood pressure less than 140/90mm Hg.
You may also perform these exercises anywhere and anytime. Just take note in your calendar if you sometimes have problems remembering. On the other hand, aerobic exercises may also improve your cardiovascular health, aside from just your male sexual health. By doing so, you can also experience an improved capability of achieving and maintaining erection.
Taking one of these tablets will not automatically produce an erection. Sexual stimulation is needed first to cause the release of nitric oxide from your penile nerves. These medications amplify that signal, allowing some men to function normally. Oral erectile dysfunction medications are not aphrodisiacs, will not cause excitement and are not needed in men who get normal erections.

Yohimbe can interact with several drugs and medications, so it’s not safe for everybody to use. Don’t take yohimbe bark if you’re currently taking any ACE inhibitor drug, beta blocker, SSRI drug, MAOI, stimulants or caffeine-containing drugs, or tricyclic antidepressant drugs. Those who have any of the conditions listed below should not take herbal treatments like yohimbe without speaking with a doctor first.
