A prospective, randomized, double blind study of 30 men showed that Tribulus terrestris was not more effective than placebo on improving IIEF scores or serum total testosterone (41). Two accounts of hepato-nephrotoxicity have been reported in young men who ingested high doses of this herbal medication (42,43).

Coffee / Caffeine.  A study found that men who consume the equivalent of 2-3 cups of coffee per day have a 39% lower incidence of erectile dysfunction than men who do not drink coffee11. Although the study showed a correlation between drinking coffee and  a lowered incidence of ED, it did not demonstrate a causative relationship.  Note that consuming larger amounts of coffee can raise stress and adrenaline levels, and can actually contribute to ED.

If you search the Internet, you’ll find lists of specific foods that are supposed to be good for erectile dysfunction.  We’ve researched this topic extensively, searching databases of clinical studies, and found no evidence that these foods provide significant benefits from men with ED.  (We suspect that most of the sites on the web are simply copying lists from one another.)
Dietary supplements are another avenue that patients can explore when it comes to treating their ED. Many products exist on the market but be warned that they may not have the scientific proof that can support claims of better erections. In one study done on multiple supplements that were proven to not work, it was found that patients found improvement in their ED 25 percent of the time. This suggests that supplements that do not work have a "placebo effect." This means that if a patient believes the medication truly works, he will notice improvements in his symptoms. This is a psychological phenomenon and can also be considered a root of why ED can occur. Depression or anxiety are two common reasons for ED and if patients take supplements that they believe will help them, they may have more confidence in sustaining an erection. [10]
By far the most common cause of ED is vascular dysfunction. When the arteries that supply the penis with blood to achieve and maintain an erection are blocked or hardened, or the lining of these arteries are damaged, blood flow is reduced. Vascular dysfunction not only affects the small arteries of the penis, but the larger coronary arteries of the heart too.
